Blepharoplasty in Perth: what you need to know

Your eyes say a lot about you, and droopy lids or puffy under-eye bags can make you look tired, older, or even affect your vision. If you’ve ever pulled your eyelid skin up in the mirror and wondered what a refresh could look like, you’re not alone.
Blepharoplasty (eyelid surgery) is a procedure that removes excess skin, tightens muscles, and reduces puffiness for a more refreshed, awake appearance. But what does it actually involve? How long is recovery? And will it look natural? Here are things you need to know.

What's covered

What blepharoplasty can and can’t do
Blepharoplasty isn’t about drastically changing your face—it’s about refining, restoring and enhancing your normal features.
What blepharoplasty can do
- Remove excess, sagging skin from the upper or lower eyelids
- Reduce puffiness and under-eye bags
- Improve vision when drooping skin obstructs sight
- Restore a more younger and refreshed appearance
What blepharoplasty won’t do
- Change the shape of your eyes
- Remove dark circles under the eyes (these are usually due to pigmentation or skin thinning)
- Stop the ageing process
Types of blepharoplasty
Not all eyelid concerns are the same. The right approach depends on whether you want to address sagging upper lids, puffy under-eye bags, or both.In certain cases, a brow lift may be recommended to assist in lifting the excess skin causing the hooding.
Best suited for drooping upper lids that create a tired or heavy look. The incision is placed in the natural eyelid crease, making scars virtually invisible. This procedure can also improve vision if sagging skin is blocking peripheral sight.
- Upper eyelid surgery i
Designed to remove excess skin, tighten muscles, remove or reposition fat that creates under-eye bags. The incision is usually placed just below the lash line or hidden inside the eyelid (transconjunctival approach) for a non-visible scar result.
- Lower eyelid surgery i
A full eyelid refresh that targets both sagging upper lids and under-eye bags. This option is often combined with a brow lift for a more lifted, normal look.
- Upper + lower eyelid surgery i
- Double eyelid surgery (East Asian blepharoplasty) i
There is one main technique:
- Surgical incision technique – More permanent, with better long-term results.
Blepharoplasty is often combined with brow lift for a more balanced facial enhancement.
A consultation with Dr Tuan Pham will help determine which approach is best for your concerns.

What happens during eyelid surgery?
Blepharoplasty is a procedure, usually performed as a day surgery under local anaesthetic with a general anaesthetic.
- The incisions are placed in the natural folds of the eyelid (upper) or just below the lash line (lower) to ensure minimal visible scarring.
- Excess skin and fat are removed or repositioned to restore a smooth contour.
- Fine sutures are used to close the incisions, and tiny dressings may be applied for the first few days.
- The procedure typically takes 45-60 minutes for either the upper or lower lids, and 90 minutes if both are treated together.
What about scarring?
Scarring is minimal and well-hidden. Upper eyelid incisions are placed in the natural crease, while lower lid incisions are positioned just under the lashes or inside the eyelid. With proper aftercare, scars fade significantly over time.
To further minimise scarring, avoid direct sun exposure, use scar creams as recommended, and follow all post-op care instructions.

What to expect after eyelid surgery: post-recovery timeline
Swelling and bruising are at their peak – cold compresses and keeping your head elevated will help.
Tiny dressings may be placed in the corners of the eyes for 5-7 days.
Sutures are removed after 5-7 days.
Most swelling subsides.
You can return to work and light activities.
Final results become visible as the eyelids settle.
Scars continue to fade.
What are the risks?
Modern blepharoplasty is safe, but all surgeries come with risks. Dr Pham follows the highest standards to reduce complications, but some issues can still occur.
General risks of surgery:
- Wound infection (may require antibiotics)
- Bleeding or blood clots
- Sore throat from the breathing tube
Specific blepharoplasty risks:
- Temporary swelling, bruising, and tightness
- Dry or watery eyes—may require artificial tears
- Eyelid asymmetry (minor differences are normal)
- Surgery may lead to some sagging of the lower eye lid, which might need to be corrected later
- In rare cases, difficulty fully closing the eyes if too much skin is removed
- Very rare: vision loss (1 in 40,000 cases, often linked to heavy lifting, coughing, or pre-existing eye conditions)
Contact Dr Pham’s office if you experience:
- A fever above 38°C
- Severe pain or excessive bleeding
- Vision loss or significant redness and swelling
As with any surgery, results and recovery will vary. Dr Pham will discuss individual risks, benefits, and expectations in your consultation.

Real talk: is blepharoplasty worth it?
If your eyelids make you look tired or interfere with your vision, this surgery can make a huge difference. For most patients, results are long-lasting, scars tend to be minimal, and recovery is generally straightforward — but individual experiences can vary.
Most patients say they look refreshed—not “done”—and that they feel more confident in their appearance. If you find yourself constantly lifting your eyelids in the mirror or using makeup tricks to fake an eye lift, it might be worth considering.
