What is the earliest age for otoplasty?
For children, prominent ears can attract teasing, which is why some parents choose to have the procedure done early. Otoplasty can be performed from the age of six, as the ears have reached nearly full adult size by then. A key benefit of performing the procedure in children is that the cartilage is still soft and easier to reshape. However, otoplasty remains highly effective in teenagers and adults, even though the cartilage becomes firmer with age.
Why do some ears stick out?
The shape and position of your ears are mostly determined by genetics. Some people are born with ears that protrude more than usual because the natural folds of the cartilage haven’t developed properly. In other cases, the ears may be larger or more asymmetrical than desired.

What happens in an otoplasty procedure?
Otoplasty is an operation that usually takes between one to one and a half hours. It is performed under local or general anaesthetic for adults and teenagers, while children typically have the procedure under general anaesthesia.
- A small incision is made behind the ear, hidden in the natural fold where the ear meets the head.
- A portion of skin may be removed to access the cartilage.
- The cartilage is sculpted and repositioned into a more natural shape, then stitched into place.
- In some cases, a portion of cartilage may be removed to allow the ear to sit closer to the head.
- Stitches along the incision line will either be dissolvable or removed within seven to ten days.
- Most patients go home the same day, though young children should be monitored by a responsible adult for 24 hours.
The goal is a natural-looking result—not an overcorrected or “pinned” appearance.
What about scarring?
One of the biggest concerns with any surgery is scarring. The good news is that otoplasty scars are minimal and well-hidden. Since the incision is placed behind the ear, it’s practically invisible once healed.
With proper aftercare, including scar creams and avoiding sun exposure, any faint scars will fade significantly over time.



What to expect after surgery: post-recovery timeline
Healing after otoplasty is straightforward, but patience is key. Here’s what to expect:
Swelling and mild discomfort are normal. A protective headband is worn day and night to support healing.
Most swelling subsides, and many people feel comfortable going out in public. The headband is now only needed at night.
Ears start to settle into their final position. Any residual swelling continues to improve.
Full results become visible as the tissues fully heal.
Most adults can return to work after five days, while children should wait about a week before going back to school. Children should avoid rough play and sports for up to six weeks to protect their healing ears. Earrings should not be worn for several weeks after surgery, and wearing a hat and sunglasses can help protect the area from the sun.
What are the risks?
Otoplasty is a safe procedure, but like any surgery, there are some risks to be aware of. Dr Pham follows the highest standards to reduce complications, but some issues can still occur.
General risks of surgery:
- Wound infection, which may require antibiotics
- Bleeding, blood clots, or seroma (fluid build-up)
- Chest infection
- Sore throat from the breathing tube used during anaesthesia
- Scars that may become raised (hypertrophic or keloid)
Specific otoplasty risks:
- Mild asymmetry—no two ears are identical, but in some cases, minor revision surgery may be recommended
- Re-protrusion in about five percent of cases, where the ears may slightly return to their previous position
- Temporary reduced sensation around the ears, which typically returns to normal within six months
- Temporary nausea or loss of balance
- Skin necrosis, which is extremely rare
